The following required information is at the request of our clients as per their contract with PharmaTech Staffing, Inc. As per their protocol, all forms need to be filled out by PharmaTech Staffing employees that will be working at Client Facilities.

Each facility requires the HIPAA Security Policy and Meth Defense Training be understood and signed off with the understanding that all employees are familiar with said policies.

Those working at Rite Aid/Eckerd or Genuardi’s must also click on their tab and follow the additional procedures required to work at these facilities.
